The PHQ-9 is a 9-question survey that allows you to measure and track your depression symptoms over time. It takes less than 3 minutes to complete. Simply answer the 9 questions honestly and to the best of your ability. After you complete the survey, you will receive a score that measures your current level of depression. Taking the PHQ-9 survey regularly (recommended at least bi-weekly) will allow you to measure the impact that your activity has had on your mental health over time.
This app is intended to be used in conjunction with visits to a health care provider. Only your health care provider can properly diagnose, measure, and treat symptoms of depression. Your responses to the survey are completely private. No one will see the results of your survey unless you choose to share them with your health care provider. All your data is encrypted so no one will ever be able to identify your activity or responses.
